Use the Member Status Change Form (88 kB PDF) if you're going out-of-town and moving away from the area. After filling out this form, please date it, sign it and fax it to 734-944-9310. You can also submit the same information required on the Member Status Change Form in the body of an email message and send it to: information@uofmflyers.org. With either option, please be sure to include the reason for your request to change membership class. In some instances there are restrictions and change criteria required by the Operating Rules, therefore a review of the reason for the request is required. Please do not leave the Member Status Change Form at the office.

FAA Form 8710 (344 kB PDF) is required at your practical test for a new rating.

The Proxy Voting Form (41 kB PDF) is used at Membership Meetings. If you can not attend a Membership Meeting, please fill out a Proxy Voting Form and give it to someone who will be attending the meeting. This form enables that person to vote on your behalf. The By-Laws state : "At any meeting of the members, the presence in person or by proxy of two tenths (2/10) of the members with the right to vote at the meeting shall constitute a quorum." We need your presense or your proxy so we can vote.
